Santizo Health + Rehab Centre provides chiropractic, physiotherapy, massage therapy, and related rehabilitation services at our clinic located in North York, Ontario. All services are provided by licensed and regulated health professionals in accordance with their respective governing bodies in Ontario.
2. Appointments and Bookings
Appointments may be booked through our website, by phone, or in person. By submitting a booking request, you agree to provide accurate and complete information. We reserve the right to decline or cancel appointments at our discretion.
3. Cancellation Policy
We ask that you provide at least 24 hours’ notice to cancel or reschedule an appointment. Late cancellations or no-shows may be subject to a cancellation fee. Repeated no-shows may result in restricted booking access.
4. Health Information
You are responsible for providing accurate and complete health and medical history information prior to treatment. Santizo Health + Rehab Centre is not liable for any adverse outcomes resulting from incomplete or inaccurate information provided by the patient.
5. Payment
Payment is due at the time of service. We accept cash, debit, and major credit cards. Direct billing to insurance providers may be available for select plans. It is your responsibility to understand your coverage. Any unpaid balances remain the responsibility of the patient.
6. Limitation of Liability
Santizo Health + Rehab Centre and its practitioners are not liable for any indirect, incidental, or consequential damages arising from the use of our services or website. Our liability is limited to the maximum extent permitted by applicable Ontario law.
